The Critical Time Intervention (CTI) model is a new approach for Fulfilling Lives Newcastle Gateshead. From June 2018 we have offered this different approach to people on our caseload who are experiencing a transition, such as leaving prison or hospital.
CTI was designed to break the vicious cycle between institutionalisation and homelessness, and to promote stable community living by providing people with mental illness with emotional and practical support and strengthening their long-term ties to the community during transitions from institutional to community living.
We support people through CTI who are experiencing a range of transitions, including prison release, hospital discharge, an accommodation move, discharge from rehabilitation and moving from children’s to adult services. Once identified for CTI our practitioners discuss the model with the person, explaining that we will provide a structured 3-phased 9 months intervention following transition which will have two clear aims:
